Chronic lung disease can be a tremendous burden for those living with the disease and their loved ones. In 2016, patients made more than 2 million emergency department visits and experienced more than 650,000 hospitalizations due to chronic obstructive pulmonary disease (COPD), one of the most prevalent chronic lung diseases. COPD also costs the healthcare system more than $50 billion a year – mostly due to ER visits and hospitalization.
BreatheEazy gives COPD patients the option of managing their own care and the ability to optimize their quality of life. It’s designed to help patients avoid rehospitalizations – and breathe easier at home, where they belong.
How BreatheEazy Works
Patients who enroll in BreatheEazy receive regular visits from CHC staff with specialized training in pulmonary disease evaluation and management. They also receive a medication kit that’s customized to help them manage their COPD symptoms at home. In addition, patients and their caregivers receive education and support from the CHC care team. All of this is designed to increase the patient’s ability to effectively manage their disease. The result is reduced hospital stays and emergency room visits – and better quality of living for patients and caregivers!
