Notice of Privacy Practices (NPP)
Effective Date: February 16, 2026

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Our Promise About Your Information
We are required by law to keep your health information private, give you this Notice, follow it, and tell you if an unsecured breach occurs.

How We Use and Share Your Information
We may use and share your health information to provide your care, obtain payment, and support daily operations of Center for Hospice Care. We may share information with your care team, your family or caregivers involved in your care, and others as allowed by law.

• Public health and safety reporting
• Abuse, neglect, or domestic violence reporting
• Health oversight activities
• Court orders and certain legal processes
• Law enforcement purposes
• Coroners, medical examiners, funeral directors, and organ donation
• Workers’ compensation programs
• Approved research
• Fundraising communications (you may opt out at any time)

Uses that Require Your Written Authorization
We will not use or share your information for marketing, sale of information, most uses of psychotherapy notes, or other uses not listed in this Notice unless you give written permission. You may revoke permission at any time.

If we create or maintain SUD treatment records protected under 42 CFR Part 2, these records have additional protections:

• They cannot be shared without your written consent unless Part 2 allows it.
• They cannot be used in legal proceedings without your written consent or a valid court order after you receive notice and an opportunity to be heard.
• You must be provided a clear opt-out option before these records are used for fundraising.
• Once information leaves our organization, it may be redisclosed and no longer protected by HIPAA. However, SUD records disclosed under Part 2 remain protected and may not be redisclosed unless allowed by Part 2.

You have the right to:

• Get a copy of your medical record (paper or electronic)
• Request corrections to your record
• Ask us to communicate with you in a specific way or place
• Request restrictions on certain disclosures
• Receive an accounting of disclosures
• Get a paper copy of this Notice
• Receive notice if a breach occurs
• Opt out of fundraising communications

Our Duties
We must protect your information, follow this Notice, and update it as needed. When other laws offer more protection, we follow the stricter law.
