COMPANY PROFILE
Established in 1978, Center for Hospice Care is an independent, community based, not-for-profit organization, improving the quality of living through hospice, home health, grief counseling, and community education.

We have four offices throughout North Central Indiana.  Our newly completed main campus is located in Mishawaka, which adjoins South Bend.  Located on the St. Joseph River, the campus has become the crowning jewel of the area’s renovation. Included is the brand new Ernestine M. Raclin House, along with Administrative and Clinical buildings, the Hospice Foundation, and Center for Palliative Care’s freestanding clinic.  Ernestine M. Raclin House provides 12 patient rooms, all of which face the river. Esther’s House, our other inpatient unit located in nearby Elkhart, houses area clinical staff and provides seven beds for patients dealing with pain and symptom issues. It is located on a beautiful 12-acre campus with peaceful gardens and woods.

PARTNERSHIPS

Also hosted inside the Hospice Foundation is Global Partners in Care.  A globally recognized program that partners North American hospices with hospices throughout Africa as well as India and Nepal. Center for Hospice Care has partnered with the Palliative Care Association of Uganda (PCAU) since 2008. Two internationally award-winning documentaries have been produced by the Hospice Foundation that are used to help support the program.

Education is also imperative to the core values of Center for Hospice Care. Center for Education & Advance Care Planning was created to inform the community about end-of-life options, to normalize the conversation, and to help individuals complete those wishes prior to a crisis occurring.

IU Talk, based on the highly respected VITALtalk program, gives clinicians the constructive communication skills they need when navigating conversations about serious illness. Research shows that effective communication reduces unnecessary treatments and suffering for patients and their loved ones. It also boosts patient-centered care.

To date, IU Talk has held over a dozen multi-day courses for fellowship and residency training programs. These programs include critical care, oncology, nephrology, cardiology, and internal medicine. In addition, over 200 faculty physicians have participated in the program’s half-day sessions. This includes physicians and advanced practice providers from Internal Medicine, Family Medicine, Pediatrics, Surgery, Obstetrics-Gynecology, and Neurology.

The vision of Center for Hospice Care is: To be the premier hospice and palliative care organization for all end-of-life issues.
