The care you’re receiving from specialists and your primary care provider is essential, but you may not realize there are additional services that can help manage symptoms, improve comfort and quality of life, and help out your family too.
The official term for this type of care is “palliative medicine” (but who can pronounce that?). It’s sometimes called symptom management, advanced illness care, or serious illness care. Think of it as another type of medical specialty that focuses on YOU, not your disease. Why not get the most complete, comprehensive care possible?
Our Kaleidoscope palliative care program does just that. And referral to the program is simple – one call from your provider to 574.367.2469 is all that’s needed to begin the process. Once we have your records, our team will schedule an in-home appointment with you to provide a comprehensive palliative care assessment that addresses your physical, psychological, social and spiritual needs. It also includes an assessment of your caregiver’s needs and concerns. Once you’re enrolled in Kaleidoscope, our staff will coordinate care with your provider(s) to assure your needs and your caregiver’s needs are being met.
We also offer community-based referrals to support groups and grief/bereavement services as needed. Our staff will cover goals of care conversations and advance care planning, including the completion of POST (physician orders for scope of treatment) forms with you as well. Our experienced, compassionate staff will talk to you and your caregiver about your disease processes and progress.
