Those who are getting older or living with a serious or chronic illness, are often receiving care from a primary care doctor and one or more specialists. This care is essential, but many providers, patients and caregivers don’t realize there are additional services that can help manage symptoms, improve comfort and quality of life, and help the patient’s caregiver, too.
That’s why Kaleidoscope was created. Kaleidoscope is a home-based palliative care service that offers people with serious advanced illness the right care, at the right time, in the right place. It’s a wrap-around program that enhances the care given by primary care provider and specialists. It also provides support for caregivers and loved ones.
