Center for Hospice Care (CHC) is accredited by the Community Health Accreditation Partner (CHAP). Any questions or complaints that are not addressed to an individual’s satisfaction by CHC may be addressed by calling the CHAP toll-free hotline number at 1-800-656-9656. The hotline operates 24 house. If voicemail answers, please leave a message and your call will be returned. The purpose of the hotline is to receive complaints or questions about local home health/hospice agencies and to lodge complaints concerning the implementation of advance directive requirements.

Center for Hospice Care Administrative Offices

Center for Hospice Care’s offices are located at 501 Comfort Place, Mishawaka, IN 46545, along with the other administrative offices of the Hospice Foundation and the Joel and Elizabeth Welch Life Transition Center. Ernestine M. Raclin House, our 12-bed inpatient care facility, is right next door at 535 Comfort Place. You’ll find the Center for Palliative Care at 211 N Cedar Street, on the eastern edge of our campus.
