Dementia Care
Dementia affects thousands of families in our community every year. At Center for Hospice Care, we address the unique needs of patients affected by advanced Alzheimer’s disease and dementia – as well as their families – through our
Dementia Care Program. This program offers:
- Comprehensive, team-based model for care delivery
- Complete care—physical, cognitive, emotional, and spiritual support
- Support for family and professional caregivers
- Dementia care training for staff and volunteers
What to Expect from our Dementia Care Program
Highly personalized care plans
We tailor care plans to each patient’s preferences, using information provided by family members, the patient’s physician, volunteers, social services, and nutrition professionals.
Pain and symptom management
Our pain management services help to keep a patient comfortable, even when the patient may be unable to communicate their pain. Specialized training helps our team care for physical symptoms common among awareness impaired patients.
Trained volunteers
Every volunteer receives training in dementia care to provide appropriate care and comfort, based on the patient’s preferences.
Care wherever you call home
Volunteers can provide support in the home, at a long-term care facility, in the hospital, or in an assisted living community.
Complementary Comfort Care
Complementary Comfort Care volunteers use caring touch, massage, and aromatherapy to help comfort and connect with patients.
Assistance available 24/7
Our team is always available to help comfort, educate, listen and understand everyday.
Free education and resources
Families and caregivers can learn more about dementia and Alzheimer’s disease through our literature and other free resources.
For those facing cognitive disorders that are not ready for hospice care, we also offer service through Milton Adult Day Services. Milton provides a safe environment for individuals needing assistance during the day time hours where caring, warmth and compassion encourage each one to flourish.
