HeartWize gives patients with chronic heart disease the option of managing their own care and the ability to optimize their quality of life. It’s designed to help patients avoid rehospitalizations – and stay home, where they belong.
How HeartWize Works
Patients who enroll in HeartWize receive regular visits from CHC staff with specialized training in heart disease evaluation and management. They also receive a medication kit that’s customized to help them manage their cardiac symptoms at home. In addition, patients and their caregivers receive education and support from the CHC care team. All of this is designed to increase the patient’s ability to effectively manage their disease. The result is reduced hospital stays and emergency room visits – and better quality of living for patients and caregivers!
